It's not a perfect solution, but I was having this problem a lot until I started putting in walls/doors to divide every toilet/shower/bath. If one of my sims is still stupid enough to follow someone into the bathroom to try to chat, I can't always stop that, but at least they're unlikely to get embarrassed from trying to pee or shower at the same time.
I'm liking this idea. So you place a wall within the room where the toilet is - like a divider?
