It seems like every randomly generated outfit for townies includes the hearing aids (in any outfit), the durag or bonnet (especially on white sims), or the nature's calling sandals (in the swimwear category). I don't mind it every once in a while, but it does get annoying to remove every time. I do have mods but none that would influence the townie's outfits.
I agree with this (even as a white person who wears a bonnet at night due to my wavy curls). It's wild how un-put-together these outfits are. And people who need hearing aids don't just wear them for one outfit category.
As far as mods go, there's one on Curseforge that I like that helps (when I play with mods): No Make Up No Accessories on Randomized Sims. I think it's by SimplyAnjuta but just doing a search for it should get you to it. I realize you were saying that mods aren't influencing this problem, which is true, but at least there is a mod that helps! Good luck!
