Hi All!
My name is Amara, and I have been a dedicated Sims 4 player for years and really appreciate the game's depth and commitment to diversity. However, as we approach 2026, The Sims 4 still does not represent disabilities in its Sims, build/buy mode, or gameplay. As someone with an invisible disability, I would love to see cognitive disabilities included in the CAS personality features. While I can configure my Sims with traits that reflect my own cognitive disability, there is still a complete lack of physical disability representation in the game. Why aren't there options for wheelchairs or canes, especially for older Sims? Modular ramps in build/buy mode could also accommodate these needs. Disabilities appear in many different ways, and I believe Maxis developers are capable of representing them, especially since modders have already addressed this gap. If the Sims Team wants to uphold its commitment to inclusivity, disability representation should be included in The Sims 4 base game so all players feel truly represented.
I'm also curious to hear from those with a disability (visible or invisible), if willing to share, what your stance is, and how you would like to see it represented.

I mean, yeah. The sims is all about people and stories, disabilities should definitely be included - it’s a part of our living.
I can empathise particularly with those who are disabled and play to create their own little world for themselves, knowing how limited the CAS options are because in the sims world everyone appears “able”. No physical signs, no mental signs, just straight up sims. We can be a whole ‘nother species but we can’t be disabled! As far as I know the only representation they have is hearing aids for the hearing impaired.
We have scars and stretch marks (correct me if I’m wrong) but no fake or missing limbs, no canes or walking sticks I agree, there’s not even much going on in the medical and therapy field (we have hospitals for illnesses and birth and a relationship counselling career, both requiring packs to access).

More object ideas along these lines could be:
Shower chairs, mobility scooters, and weighted blankets
More Traits Include:
Extra rest need, Frequently sore, Chronic pain (with mood effects), Anxiety (with mood effects), Tinnitus (with tense moodlet), Bipolar (random very happy and sad moodlets), and Frequently fearful.
More CAS items:
colostomy/ ileostomy bags that have to be emptied, wigs for people with alopecia, crutches, support boots, and casts.

I love the idea of these extra traits but I actually think they should add sim challenges - the same way we have lot challenges. Like, vegetarian or lactose intolerant are not traits (I'm a vegetarian myself and nobody has ever said it's a part of my personality). I feel like traits should be actual things related to personality - e.g. lazy is a trait but if they add something like chronic fatigue it should be a challenge - I think the best way to describe it is things that impact gameplay vs things that impact interactions
