Just to eliminate the possibility, does your lot have a challenge enabled that might attract gnomes?
No, I’m playing in San Myshuno. No lot traits that have to do with gnomes at all. My sims did get those ridiculous ailments, but I cured them and the gnomes still come.
I really wish there was just a switch like there is for dust bunnies
