Can't get a guaranteed diagnosis

My sim is level 5 in the doctor career and has logic skill level 9. Whenever I try to preform tests on my patients all of them either come back as inconclusive or simply suggest that there is an ailment and that I need to do more testing. I have tried all the tests every time to the point the patient starts losing relationship points with my sim and nothing happens. Does it just become a guessing game and I have to guess what to diagnose the patient? My sim is also very focused every time I send her to work. I just can't get a guaranteed diagnosis. Also I noticed, sometimes she continuously fails to get a swab sample from patients even when the environment is clean, my sim is happy and super focused. Not sure if I'm doing something wrong, or if EA gave 0 cares about this expansion ( I know its very old lol).

    As in real life it can be hard to come with a diagnosis but usually there are signs you can look for. For instance Swatting in front of eyes occasionally = starry eyes or Giggling = Gas and giggles.

    @SkarlatouMia  When I play the Doctor career, I admittedly use this guide Doctor Career: Symptoms and Illness List I have found that once in a while something is not correct, I usually think that's a glitch.

    I also move my camera to keep an eye on the patient I am running tests on. If my Doctor Sim needs to analyze the sample from the patient, I watch the patient to see what symptoms they display. I also tend to gravitate to patients that show symptoms as they come in.

    I can usually get it narrowed down to 2 or 3 diagnosis. I don't remember getting guaranteed diagnosis but a few times.
