While I went and got this, at the same time I checked the GEForce Activity and it says 1 program - Sims 4.
Only thing I can think of is that your Nvidia card has a different VENDOR and DEVICE ID than what is on the GraphicsCard.sgr file. More than one vendor can make the same graphics card. If it's not on that file then in your case that config.log file picks up the Intel card by default using the low settings.
However, as long as the Nvidia GPU Activity is reading 1 program then it doesn't matter what the config.log file says. The game is actually using the gaming card.
