First Loss Milestone not appearing

Today I was playing and noticed that when my elderly sim dies neither his wife (also elder) or his daughter (young adult) got the First Loss Milestone. They are both present on the home lot with him at the time of the passing and the wife did get the Widowed Milestone. I tried to saving after the death to see if the milestone would appear when reloaded but it did not. I am also not playing with any mods.
