Do you have Eco Lifestyle? Could it be that the toilets have been upgraded to be composting toilets? Composting toilets will catch on fire if you don't remove the compost. The compost is meant to feed the bugs in the Insect Farm.

It's likely the upgrade as DizzyDee said
That said: Do you have enchanted by nature and bad luck ? That could be the reason why it's occuring more often

I am having the same issue and it just started today. I have played this same family since adding the most recent expansion without this issue and I have not made any upgrades to the toilet. very frustrating. i have started spending all my aspiration points to get everyone a steel bladder which is better than everyone in my family dying off. Any help?

I had this happen last night but thinking about it I have enchanted by nature and my sim is a naturapath and treats patients at home. A lot of patients come with gnome lure affliction and I did notice before this happened that a repair gnome repaired and upgraded my items. I haven't checked yet but pretty sure that was the reason.
As you can see that was the reason.

I have a toilet in my house that is not pranked and yet keeps setting my sim on fire when they use it. I have even replaced the toilet in buy mode, and it still keeps getting set on fire.

Hi, you may have the compost upgrade which causes fires if the compost is not regularly emptied!

Could you please provide the following information
- Are you using mods/cc in your game ? Please remove them and test without them if you do.
- What platform are you playing on (PC, Mac, Playstation, XBox)
- It's usually the compost upgrade so did you install that yourself or did you have gnomes upgrade everything on your lot ?

There's several issues with stuff catching on fire.
There's three potential causes for toilets
- Cursed book event on the lot (can affect any object)
- Dust or some unknown factor that makes objects combust into fire
- Most likely for toilets: Compost upgrade.
For the compost upgrade the following info is needed so your post can be merged with the correct thread
- Did you upgrade the toilet yourself ? If so, do you have EBN and luck turned on ?
- If not, please check the toilet if maybe Gnomes upgraded it for you.
