Make sure at least one of the household Sims is awake if you want to save the game in Pause mode. If you save the game with everyone sleeping, the clock will start automatically when you reload the save, even if you had it paused when you saved. This also happens if everyone is at work or at school.
I usually pause the game, with at least one Sim awake and at home, before I save. The game remains paused when I reload it. That's without mods, which I have never used.
