Sim Freezing to Death while wearing Warm clothes

When my ranchhand froze to death, I didn't think anything of it, I just figured she wasn't wearing warm clothes. BUT then my sim started freezing to death even tho she was also wearing her warm cloth...

Same thing happened to my sims. I was playing with a household at a park and a sim from another household froze to death and then my current sims began to freeze to death even though they had their cold weather outfits. I had to bring them inside for it to go away. Has this happened to you again?
