Sims Survey Error

I asked EA support this question, and after they investigated it, I was told in their chat to ask in this specific forum instead even though it's not directly related to gameplay.

I received the email "Take a survey to earn a $5 EA App coupon" and did the Sims 4 survey, but on the coupon code page, only a little rectangle showed up, and there was no code. Then on the last step of the survey, I clicked "Next" and got an error message. It was a very time-consuming survey, and I'm concerned my answers didn't go through at all? Did this happen to anyone else? I took screencaps of both errors and have attached them here.

    I was sent here as well after contacting an EA Advisor. I was able to see the code and screenshot it but when I attempted to redeem it I just got a message stating "Your code cannot be processed at this time. Try again later.".
