Why did my Sim's appearance change completely?

Why did my Sim's appearance change completely when it went to sleep? Her hairstyle, face, outfit... everything changed. The only thing that remained was her hair color. She even went from adult to young adult. I exited and re-started the game several times, and it still occurred. This is so frustrating. What can I do to prevent it?

    Go back to Create A Sim (CAS) and change her Sleepwear hairstyle, clothes, and makeup to match the one you want to see. 

    Sims change outfits to match what they are doing, and their icon changes to match the outfit. The outfit includes clothes, hair, makeup, accessories, and anything else that can be changed from one outfit to another.
