Yes, the cake has to be in the dead centre of the table it is placed on for the add candles/topper option to work. Unfortunately, when the Sim makes the cake, by default they always put it down on the table slot that's nearer the front edge, so you have to drag it to the middle to add candles - before anyone eats it !
I actually had these same cake blunders before and made a second cake during a party when I had already brought one yet had set it down where I couldn't add the candles. Then after I had already baked the second cake and I repositioned the first cake, I could add them again. Pausing is your friend!
