For me its jaw lines and ears. Most of my sims get much smaller ears than what they typically default to because it feels to me like the default ear is way bigger than the average person's irl. Proportionally they just don't work for me.
Jaw lines are odd for me. I tend to give my female sims square jaw lines and high cheek bones while I give the males a much softer jaw line and cheek bone area. I just like the look of them better. No idea why.
