That not doing anything at all will mean it reverts to defaults. That's how, literally, everything else in CAS works. Other than inputting a name you don't have to change or alter a single thing if you don't want to and you still get a perfectly usable Sim.
As ever these things all boil down to options. Much could be solved by a player being able to go into options and set the default pronouns desired by the player for all new sims created by the game. A detailed options screen would be a boon to the game, IMO.
