Diaper thought bubble

Will someone please tell me what the diaper in a thought bubble means, NOT go to the bathroom.

Are your older sims getting it? IIRC a diaper thought bubble has something to do with the Happy Toddler trait, when you have a toddler age up after getting their skills to at least level 3.
