Players shouldn't be forced to have such a realistic feature brought into the game. I kinda imagine it would be like having transgender sims: you can have them, but you don't have to have them. Also, as has been pointed out numerous times, mental issues are not even hinted at as being on the table.
I have no earthly idea how to handle it, but that doesn't meant I'm opposed to it. As an able bodied person, my opinion doesn't matter at all, beyond 'I hope the people who want this are pleased with the result.'
