I think they would be rather difficult to include. How would you animate it? What do you include? @Cinebar is right. It would be fine if the pumped up Get to Work with some added fictitious illnesses and such, but I wouldn't add things like down syndrome, etc. The game is supposed to be a spoof on reality. Not reality itself.
I think it would be hard to do even respectfully. Not everyone would be keen on the idea. How many of these threads have gone down in flames? I would say most, if not all. You have to think how far are you ready to go. How would the game handle all of the special animations throughout the game. It is not the same as being gay, straight, bi, or trangendered. The animations for them are all the same. And the sims have been essentially bi since day one anyway. It was up to the player to assign a sims preference, and even then through gameplay, that could change.
