Disappearing Relationships

Lately I've noticed times where two Sims have begun having a relationship, sometimes with about 1/4 friendship and romantic feelings, then either the following night or two nights later their entire relationship has vanished. It's like they never even met. To stop this, I have to do the "Ask to be Girlfriend" action in that one playing session.
