I agree with @DaWaterRat about Sims recognising certain items rather than the room itself. When you use BB mode, you can select furniture by room type, (i.e. bedroom furniture includes beds and dressers, living rooms include sofas etc.). So if the Sim asks for a bathroom renovation and you renovate a room using no items from the bathroom category, they may not be happy.
Also, counters/sinks fall under both the kitchen and bathroom category, which may be why Sims wash their hands and prepare food in the wrong room sometimes.
I know nothing about coding but this is just a guess.
