I usually use a mod that adjusts the calories in sims food to have a more realistic effect (the problem with the default game is that it doesn't account for the fact that real humans have a base metabolism use energy and calories to just, like, exist, and therefore even the lowest calorie foods will cause significant weight gain without a fair amount of exercise cause those calories aren't getting used the way they would by a real life person), so that allows for more variety of lifestyle and body shapes. Sims will still gain weight if they eat a lot of heavy foods and never work out, but instead of every sim that doesn't work out a lot ending up looking the same, there's more of a spectrum, so I feel more okay following each specific sim's inclinations when in comes to physical activity and just letting each body do it's own thing.
