In my first save I changed a bunch of premades, but I regret doing it. In every other save I pretty much leave them alone because I realized that my game is so much more interesting when all of the Sims look different from each other and not all beautiful. I actually really like plain or strange-faced Sims now.
But once in a while, if I end up joining a pre-made into my gameplay, I'll need to update them here and there either as they grow older or if they become Occults.
As for the random townies, I delete a lot of them but I try to salvage as many as I can so my game isn't changing them out too much. So I'll take them into CAS to give them some of the preset styles and new hairs or remove glasses and crazy makeup/accessories. If I'm going to see them around a lot, I at least want them to make sense. :D
