In Sims 4, I generally just create new toddler Sims in 'CAS' (by using the 'manage household' option) when I want a same-sex couple to have a child. I usually don't go by their genetics, so I guess it would technically be 'adoption'. I just don't like to use the actual adoption option on the PC/phone because a lot of the adoption choices are often really ugly... lol I know that sounds so mean, but it's true. It's just like how all the other pre-made sims are generally not really good looking at all. So I just do it in 'CAS' to make sure their children look the way I want them to.
That's one thing I love about Sims 4 that the other games didn't have... in Sims 2 and 3, I didn't really have an option for same-sex couples (other than adoption from the PC or phone). However, at least in Sims 3, I was able to use cheats to enter CAS. But still... it's a lot easier in Sims 4.
But yeah, I usually don't play with their genetics when it comes to having children for same-sex couples. He/she is usually an adopted child. But they're still a real family! Blood and biology aren't what makes a family. Keep that in mind!
