I can't remember the last time a sim died unexpectedly, they hardly die from old age even.
But in my game, the NPCs are dying off like crazy...per the neighborhood stories. I think it's great, my spellcasters love going to the bars on Wednesday nights, that's when it's Ghost night. I love dedeathifying adults and young adults. For obvious reasons, I won't bring back elders. o:)
