Do your sims exercise?
Do your sims jog to slim down? Get centered with yoga? Shoot hoops for fun? Swim, box, climb or spar?

It depends on the Sim; mostly. I’ve had a few career Athletes. I think the Astronauts need to be fairly active too, but it’s been a long time since I played one.
If not required for their career, many of my Sim have fitness as a low priority. They can certainly begin to get out of shape. When I notice -I begin sending them on jogs to slim back down.
The gym is a decent enough place for Sims to hang out during the day. Sometimes they workout for a bit of fun, especially outside the home.
