I get these calls in the game quite often. It only happens when you're at least friends with the other Sim and from the experience in my game the Sims were married and had at least one child already. If I said they should go ahead, the Sim always got pregnant. Most recently happened with Bella Goth in my game and she had twins (two girls). Never had an elderly Sim ask though, that's pretty cool.
Two of my sims had their first child with the "phone-a-friend lifeline from Who Wants to be a Millionaire" event.
What was perhaps weirder was when one of my sims got a call asking if she should accept a marriage proposal from a romantic interest of hers...who was already married to someone else. I didn't test what would happen if I told her to accept. I think the event's determined by randomly sampling the active sim's friend list and choosing a lucky "victim" who has a romantic interest in-game to give the call.
