Yes, the traits do make a difference. Not all Sims will get them on growing up, though - you need to end up especially high or especially low on a character value to get one. So it should, I think, not get repetitive fast.
They work like bonus traits, and your YA Sim could get anywhere from 0 to 5 of them.
They can't be added in CAS but there are cheats.
