I can't get past a 4 star clinic but that might be because I haven't made it a priority to use the vacuum cleaners yet. Something I started doing though (which is way cheaty but don't fault me please) is right shift click on the pets to make them happy so they'd be clean. It doesn't change their illness or make them less stressed but it's a quick fix for when you get tired of seeing dirty animals all day.
