Imagine you're playing one of those anti-social Sims. You know, lone wolf type. Maybe they're even mean or evil. Maybe they're even a criminal. And then they 'stumble' into fame! :D They have to deal with fans, news, etc. I mean, you could really torture a Sim with this if you wanted to.
Or how about a Sim who's terrible at flirting, clumsy, whatever. Maybe a Vampire or Alien who is really trying to stay hidden/undercover but ends up famous anyways? Or as someone else pointed out, a secret agent, lol.
