I usually only let my sims have one or two kids, as I prefer smaller families. But I do have MCCC's Risky Woohoo set to 5%, so there's always a chance they'll accidentally have more. Abductions are a possibility, too. For example, I had one sim who was only supposed to have one kid, and she ended up pregnant with twins the first time. Then she had a brief fling with a townie and ended up pregnant by him because of Risky Woohoo. Finally, she was abducted and ended up with a child from that. So one kid turned into four because the game apparently did not agree with my plans for that sim.
