Depends on what I'm wanting to keep track of, I suppose. I have a Rock Star in my game. While he was busy working on fame, I had him write (more like grind) music. (Years ago when GF came out, even using the Road to Fame Singing Career mod. my Sim, while wildly successful never ever even got nominated for the songs he licensed.) Turns out there was a way for him to be recognized. It's music only, not lyrics. So, upon one of my many restarts I had him write one song per music category on all available instruments. In order for me to keep track I created a sheet. I categorized the songs by instrument and by genre, if that's even the right word for it. I wrote down the title I gave it and I have updated it to include a nice little [] on the same line. Once nominated, I fill in that little gap. If he wins, it gets marked with a highlighter. And I use the highlighter over the song title, too. Sorry, without scanning items, I have no pictures to help illustrate it.
I also made album titles for him. As he releases an album, I also mark it off with a highlighter. They are numbered, so I can keep track of how many he's released, for storytelling purposes.
I also created a page for university. I've taken as many as seven kids through Uni at one time, all living in the same dorm so I could control them. I wrote down their Term grades to know who was doing what, and who needed to knuckle down to their studies more.
With the newest EP released, I learned in another thread the names of the different Infant's Milestones and was pleased that wasn't the complete list, so I added the ones that occurred in my own game. :) As my Infant reaches a milestone, (you guessed it) I use a highlighter to mark it off. This gives me a clue as to how well the little one is progressing. I used the same sheet to scribble down their Infant Quirks, too. I have another sheet for their food exploration, mostly since there was a bug that wiped out that list in game of what they liked, loved, disliked, was unsure of, so I would have some clue.
Beyond that, just pictures and as somebody else said above I do write about my Sims' antics.
