How does death by flies work?

I let my trash bin outside pile up. I attracted flies and roaches. I haven't had them swarm and kill my sim yet though.

Once I though almost maybe but then the waste manager NPC showed up and just cleaned everything (without my asking) and charged my sim 50 bucks!
