How the game generates Doctor Career Co-workers at the hospital?

I have manually selected 4 resident premades as doctors (1 Medical Intern, 1 Orderly, 1 General Practitioner, & 1 Chief of Staff), but always only two of them shows up. Each of them could be there, rotate when the shift changes, but only always two.

Is it possible to make all of them show up?
