I've gotten two clinics to five stars at this point, I think the best advice I can give is to go for a really high environment score in the lobby. If you can get it high enough, the patients won't even care if there's puke all over the floor.
once you've got the decorations sorted out, I think you're doing most everything else right. don't accept any patient before you see that they think the clinic looks great, and make sure to call the pet good at least one time before you treat it, even if you have to get it off the table again to do it
Oh wow a big thank you for your tips :) I might have to hide some items with environment score under stuff hahahaha. I will see what the game allows me when I play a little later. Had a busy day putting up our splash pool for summer, getting so hot now, need to be able to cool off :)
