These are random outfits given to your Sims during festivals or on other (unfortunately a lot) random occasions. I have a whole mod category aiming to fix all sorts of random outfits in TS4. I really don't understand why EA goes on adding new random outfits with each pack. A ridiculous amount of new random outfits came with Discover University:
I don't know where your random outfits come from, so I cannot suggest which mods you need. You should be able to figure by scrolling through my mods which situation is the culprit in your case. From what you describe, I suspect it's a festival outfit, but I could be wrong.
