Having no hearth rug (unless it's stone or masonry in front of the fireplace) is what should be the fire risk.
EXACTLY! Ugh...drives me crazy I can't put a rug right up to the fireplace because, growing up, that's precisely how it was set up in our house. We had a lovely cast iron Franklin stove in the family room, and a rug which sat right in front of it specifically to protect the hard wood floor.
Even upgrading to fireproof doesn't help in TS4. I swear it did in previous Sims games, though maybe I modded those too and just don't remember.
I have lived in houses with open fires all my life and hearth rugs are essential for the reasons you describe. Of course, build/buy also needs some fireguards to make sure :) I always put one of those up when the fire is unattended.
