I have lots of lag in San Myshuno also, and in certain parts of other worlds. Whiskerman's Wharf in Brindleton Bay is a problem, but the area around my sims house is fine (near the Delgatos). Same in Windenberg - the city parts are much worse than the natural areas. I don't know if it's the number of nearby lots that are within viewing distance or the level of detail in the city backdrops, but it's definitely a difference.
And yes, San Myshuno is the worst. It's not just graphics there, it's all the stuff going on at street level that you're not always even paying attention to using up resources. Uptown is the absolute worst, which I don't understand because there are no festivals there. I have tried tweaking my graphics settings and it does get worse on high, so I play on medium. But I still get a lot of dramatic pauses when I am turning the camera. I assumed it might get better if I got a new graphics card but it sounds like maybe that wouldn't help as much as I thought.
Maybe even though Sims 4 does not scale up or down to your specs, it is still important to make sure the GPU, CPU and memory works well together. Example; an GTX 460 will have an hard time keeping up with an i7 7800K etc which may increase lag on top of the lag that may be contributed to it not being optimized.
