is this a characteristic of erratic?
one of my sims has witnessed 2 deaths and she got no sad moodlet for either of them. i thought at first it was because she has the evil trait, but her mum has that trait too and she got the sad moodle...

TBH sometimes they just don't "witness it" despite being in plenty proximity to it. I've had family members miss the moodlet/recognition before despite being in the same household.
