I had to stop inviting Paka'a Uha over because they pranked the toilet every single time. I'm not positive, but I think this is related to the goofball trait.
I'm currently playing with a clumsy Sim. She randomly slips/stumbles (like other people have noted), and I once got a pop-up letting me know that her clumsy fingers had broken the computer. Other things in her house seem to be breaking more often than normal too, and I don't have any lot traits that would make this happen, so it could be related to the clumsy trait... or I could just be imagining it.
This same Sim also has the goofball trait, but unlike Paka'a, she does not autonomously prank the toilet in her own home. (Thank goodness!) She does seem to be autonomously choosing mischievous social interactions, but I don't know if this is new or not since I wasn't really paying attention before the patch.
