I think living underwater would be difficult to implement, unless the devs added a ton of underwater actions and interactions for mermaids. Although I can see the multitasking system help with this.
In the case of mermaids, the hunger need can be satisfied by catching and eating marine life. Bladder and hygiene (or hydration) aren't a problem. For energy, they would need some sort of sleeping underwater animation and maybe mermaid beds. Mersims could already talk underwater in TS3, plus the multitasking system in TS4 might enable most existing interactions to take place underwater. For fun, they would need to create a bunch of fun underwater activities so our mermaids don't get bored
If living underwater permanently were possible, I don't think it would be very fleshed out because all/most of the existing land furniture and architecture wouldn't work underwater, and the devs would only have enough time to create a suite of basic underwater objects. It would be like a whole different game altogether
