OK I have heard there is a cheat one can use before I send my doctor sim to work (or maybe it's after) So I can modify the hospital. I would love to redesign the layout of it to make it more efficien...

When at the hospital, open the console (control-shift-c) and type in "bb.enablefreebuild" (without the quotation marks). That should allow you enter build buy mode on the lot.
You may also want to use the bb.showhiddenobjects command so that the hospital specific debug items are available to you.
Be advised that the hospital must have at least the following, so whatever edits you do make sure these still exist on the finished hospital:

- 1-2 Surgery Tables
- Exam Beds
- 1 Front desk with computer and chair
- 1-2 Treadmill
- 1-2 X-Ray Machine
- 1-2 Chemical Analyzer
- 1 Snack Vending Machine
- Extra computers (away from front desk)
- 1 Toilet
- 1 Sink
- 1 Microwave and Counter
