Building muscles isn't the priority on a fat sim, at least right away. it is better to get the weight down first, creating a diet plan, and then slowly easing the larger sim into exercise machines and other physically demanding tasks so they burn off the fat and build muscles over time without causing a heart attack or hypertension in the process.
simply pacing around the block for a larger sim and being more active in general as well as eating more healthy foods can have an immediate effect. when that sim can stop powerwalking, they can start jogging around when their body is up to it. it is all about losing the fat at first so their bodies can deal with the demands of exercise a lot easier as opposed to asking a fat sim with no muscles to start lifting weights right away and potentially kill them by pushing them too hard too fast, it would be too greater shock to the body to manage. One needs to be eased into it until the body can manage the demands and adapt to what is being asked of it.
if only food in TS4 had values that determine what is great for a diet, exercising and healthy in general. but this game doesn't have any such thing. i mean you can get your sim to sholve down mac and cheese and then get them on the exercise machine and still come out looking like Brian Shaw or Martins Licis, that isn't right as you'd be eating the wrong food and building bad fats on your body that will just sit there.

Try jogging or running on a treadmill and when your Sims hunger bar is full, direct them to stop eating.
There are also foods that are higher in calories than others on TS4 so go for lower calorie options (a quick search online will give you a list).
It takes a bit of time but is definitely achievable to get Sims to slim down a bit.
