I thought it was because of spider attack (he lives in an apartment with Creepy Crawlies trait), but when I hovered over the moodlet, it showed that it was caused by poisonous ghost's belch. My sim didn't even interact with the ghost. He was just near her.
Ok then. My sim was able to fend off the poison, but that really surprised me. It seems like sims who die by poison may poison other sims, but it doesn't happen often. Just sharing my discovery.
