No I doubt anyone got fired over this. Maybe they got a warning if it wasn't intentional.
But obviously it wasn't a big deal if no one noticed the background image that was left onsite, because it means no one checked for further errors and it was left entirely up to the web designer to fix it. If it was
that serious a whole team would have double checked to ensure no other leaked images were present.
Although the damaged was done so it didn't matter either way...
