I play with aging off too. What I did was pick a day on the calendar for each of my sims and create a holiday called Emilia's birthday for example for my sim named Emilia. I put the traditions of baking (to make a cake), give gifts, and if the sim is a child I added play games. I never age my sims up, so having a birthday is just for fun.
When you use the "give gifts" option do all the sims in the household automatically exchange/receive gifts or can you decide who gives and receives? I haven't experienced the gift giving part yet so I still have no idea how it works.
I have autonomy off, so my sims don't do anything unless I tell them to do it. I only give gifts to the birthday sim, but I'm not sure what would happen with autonomy on. You could also use the "open presents" tradition and place one of the present piles on your lot. Then make the birthday sim open all the presents. Again, I'm not sure what other sims will do if you have autonomy on though.
