I've guess I've been pretty lucky. I think so far the most I've done is accidentally chosen a romance option because it was on the main dialog wheel and I wasn't paying attention because I was trying to quickly raise social. This can happen to me with mischief as well.
There have been several times I've gone to the romance option when talking to a sim I'm dating or married to, only to realize at the last second that it's actually another sim of mine that is dating that sim. So far I've either stopped myself or quickly canceled the interaction before it happened.
