That's my household, and I finally found an answer: the lampshade over sim's head is apparently symbolizing the cone of shame the pets can get at the vet. There's a whole thread of people with their pets being continuously afraid of the vet, apparently triggered by the health boost or wellness treat given by the vet. The funny thing is my dog there has never seen either, but I guess that's why it passes and isn't stuck, just sort of... occasionally appearing fear of vet, passing on its own.

In less buggy news, there's four dogs in the household I'm mostly playing, and there's a variety of fears and obsessions, although most often appearing ones are the obsession with the bathtub and the fear of stove.
