Mine was run by a mean, evil, hates children sim. I created one teen (who was treated like Cinderella and wasn't allowed to attend school as she had to look after the toddlers), then there were 4 kids and 2 toddlers.
I made the lot very trashy, with cracks in the walls, cobwebs, rubbish furniture etc. It was great fun.
I was thinking of doing the "evil" orphanage too eventually, if I can figure out how to get one running first. Any tips you guys have, I'd love to hear them. :)
