You have to keep talking to him until you have a full green bar then shortly after he will start coming out

I read from Carl's Sims 4 Guide that you can have children with Patchy. I'm not sure if you need to cheat to make it happen. Waiting for your update on the matter :) . I have an idea that includes patchy B)
