I think there’s a degree of randomness involved in death by old age (unless you use mccc). An elder is basically guaranteed a certain amount of days, but after that who knows. - I read someplace that it has something to do with the relationship that the sim dying and the sim pleading have/had. Like a spouse with a full relationship bar would have a better chance than a distant friend. That could be wrong but it seems to hold some truth in my saves. I once had an estranged daughter plead for her father and it failed, but in the next generation the daughter plead for her mother with a full bar and it succeeded. Of course, it only gave her about 6 hours (I think. It felt like about 6 hours but could have been a day) before she was dead again, but still..
- I was playing with Sacrificial's 'Possessed Child' mod some days ago and had my sims pleading for sims lives at least 5 times; it worked 4 out of 5! I was pretty surprised to be honest and I'm not sure if it was just plain luck.
- I've seen an elder sim only live a few more hours after being revived. In my game everyone was sleeping, time fast forwarded, then all of a sudden time slows, the camera pans on its own to an area close to my lot (which the game considers the whole area around my house to be my lot I've noticed), turns out a random elder was walking through there in the middle of the night and had keeled over across the street from my sims' home. I thought about just watching it (I haven't actually had Grim appear in any of my Sims 4 saves yet), but then I remembered my sim grows Death Flowers and had like 23 in her inventory, so I thought, hey, let's wake her up and send her over to Grim to give him a Death Flower and see what happens. The elder was revived of course , this was at like 3 in the morning sim time, well around 10 in the morning, once again the camera pans over to another spot behind my house, and whaddaya know, it's the same guy who had just died! He was dying again. So the Death Flower literally only bought him a few hours.
It was fine though, this was literally a townie she had never interacted with so no emotional investment, and it was actually a really beautiful scene playing out, it was Winter, there was snow everywhere, it was dark out, the atmosphere was perfect for a death to occur (if that sounds too morbid I'm sorry, but the aesthetic and symbolism couldn't have been more perfect). This hasn't really been my experience with death by old age. I have Sims Info Mod (which you can get a simple age with MCCC). But with Sims Info Mod, it will tell you, nearly down to the minute, when a sim will die by old age.
Example:
Ellie here is nearly done with her Elder stage.

Notice it shows she has 2 days left, including 0 bonus days. That's "mostly" true. In two days, she will die of old age...

Notice Sims Info Mod... She has lived for 9.28 of her allowed 11 days (10 as set by the game, and all sims get a bonus 1 day. If your sim maxes fitness, they get another 7 bonus days and if they complete the athletic aspiration, they get... I forget exactly, but over all they'll live roughly 35 days as an elder).
She is at 84.34% of her life. Once she hits 100%.. she's done for. This has proven to be accurate for me at least 9 out of 10 times. This is the same for every sim. When they age up roughly determines when they die. Now, this is only for my active sims that it is down to the minute. I don't know about sims that die out of my control. So, for controlled sims... there is no randomness involved in a sim dying of old age. It's all just down to the numbers.

Notice at the bottom, she has maxed fitness.
At anytime when fitness is maxed, they instantly get the extra 7 days. Same goes for the bonus days from the aspiration. - simsimsere7 years agoSeasoned Ace

I look at it this way: an elderly sim is already at the end of their life, but pleading could give you just enough time for the last goodbyes. Still it would be much more beneficial to plead for a younger sim facing an untimely death. - BlueSeaWaves7 years agoLegendI had it work once for a sim that my sim was “friends” with. Whenever my sim went to the gym, Brant Hecking would also be there! So my sim...decided to be a bully to him and eventually he got used to him. It was freezing though one day and he froze to death outside. So my sim pleaded to death and it worked! Brant and him hugged it out while my sim quietly picked his pocket
